All subcontractors working on ServiceMaster projects must submit a certificate of liability and worker’s compensation insurance naming ServiceMaster as Certificate Holder.

You must also supply a copy of your occupation license, professional license if held, and W9. You will be mailed a form 1099 indicating the amount ServiceMaster has paid you during the previous year.
